The speech behavior is characteristic of what is called global aphasia and is not particularly amenable to treatment, even in the early months following the stroke! In such cases, speech wont recover; especially after 10 years!I would hope that the speech pathologist had taught her to use meaningful, representational gestures so she could communicate her most basic needs! An example would be for her to pretend shes holding a cup and bringing it to her lips in order to tell others thats shes thirsty!Another option would be a simple communication board with drawings representing objects associated with basic needs! All she needs to do is point to the desired object! A toilet, a fork, YES/NO, bed, chair, wheelchair, TV, radio, numbers, (letters are of no value), shower, sweater, pillow, blanket, favorite foods, family and pet photos, cup, milk carton, comb, nurse/doctor, pill bottle, and toothbrush are all very helpful!Put the pictures on a 14×10 card and teach her to scan to find the desired object, just as you scan when you read! Have the real object handy so she gets the idea of matching the object to the picture (yes, even this can be very difficult for a stroke patient)!Good luck!
